On the "Side Effect Assessment" page, fill in "Medication and Surgery","Past Medical History" and the current symptom description. 2. Optionally select the symptoms label that has occurred and follow the prompts on the page to select the severity description to obtain a more granular assessment. 3. Click `Start Assessment` to view the risk level, risk score,`assessment basis`,`CTCAE grading` and personalized recommendations. 4. If the result is high risk, the 'Contact Medical Team' button will appear on the page, which can be used to record the collaboration request. 5. Open History to view past assessments and filter by All/High Risk/Medium Risk/Low Risk.
