Convert Markdown files into beautiful multi-step questionnaire forms. Support 7 types of question types including single choice, multiple choice, text, matrix, and Likert scale.
Lazy cat oidc login has been integrated. or default account admin/admin md-survey is a self-hosted questionnaire system, and the core concept is to use Markdown to prepare questionnaires. You only need to write a '.md' file and upload it to the management backend. The system will automatically parse it into a beautiful multi-step form that supports progress bars, conditional logic and other features. ** Application scenarios **: Satisfaction surveys, user feedback collection, internal voting, event registration form... Any scenario that requires structured collection of answers. ###Step 1: Log in to the management background 1. Open the application and automatically jump to the login page 2. Click the **"Log in using Lazy Cat Micro Service Account"** button (recommended) 3. Click **Grant Access** on the Lazy Cat authorization page 4. Automatic login is successful and you enter the management background ###Step 2: Write the Markdown Questionnaire Create a.md file using any text editor, such as feedback.md: ```markdown --- id: customer-feedback lang: en required: Q1, Q2 thank_you: "Thanks for participating! In the management background, drag and drop the `.md` file ** to the upload area **, or click Browse to select the file 2. The system automatically parses and generates questionnaires 3. The newly created questionnaire appears in the questionnaire list ###Step 4: Share questionnaire links After the questionnaire is released, the sharing link format is: ``` https://Your domain name/s/customer-feedback ``` Just send this link to the person who needs to fill it out. ###Step 5: View the data - Click on the specific questionnaire in the management background to view all submitted responses - Support **CSV export ** for further analysis in Excel - There is also a built-in ** data analysis panel ** that contains timeline charts and statistics
